About Us

Casse Community Health Institute's mission is to provide personalized, high-quality care on an as-needed or preventative basis. We have created a practice that we believe in and choose for our own family members.

We are an FQHC with a Patient Medical Home philosophy. We have dedicated, experienced physicians who believe in working with our patients to maintain and improve their health. We work together to serve your entire family for all of your medical needs through all stages of life. Our physicians believe in providing comprehensive health care services to our patients in a friendly, relaxed atmosphere. We also believe in educating our patients as we work with them to achieve their best physical state possible.

The Council for the Advancement of Social Services and Education (CASSE) is a 501(c)3 non-profit organization that identifies, designs, implements, coordinates, and evaluations programs in order to improve the quality of life in the communities served with dignity and respect by providing a continuum of care.

CASSE developed healthcare as one of the organization's focus areas. It established the development of the following:

  • The Community Health Institute provides access to medical health care services to improve the quality of life by focusing on the improvement of the rural medically underserved and uninsured in Louisiana health status.

  • The Community Health Institute projects were established to address the lacks of health care services in rural Louisiana as the lack of accessibility to health care services and the health care professional shortage severely limits accessible to affordable health care services in Louisiana.

  •  The Dental Health Institute provides access to dental health care services to improve the quality of life by focusing on the access of services to the medically underserved and uninsured in Louisiana health status.

  • The Dental Health Institute projects were established to address the lack of oral health in Louisiana as the lack of availability to dental health care services and the dental health care professional shortage severely limits affordable dental health care services in Louisiana.

  • The Youth Department Institute provides access to behavioral health and social service to improve the quality of life by focusing on the access of services to the medically underserved and uninsured in Louisiana.

  • The Youth Department Institute project was established o address the lack of behavior health services to assisting families in need of family support when dealing with behavior health issues.

 

The Goals:

  • Implement creative approaches to caring for the "whole" person.

  • Increase the access to health care for Louisiana’s medically underserved and uninsured families.

  • Make significant contributions to creating healthy communities through advocacy and other initiatives.

  • Strengthen current programs and expand into new locations and services.

  • Promote a community environment filled with hope, dignity and mutual respect.

  • Develop collaborative partnerships with governmental agencies and community organizations to enhance access to all services in   Louisiana.

  • Provide health care services in areas with health professional shortages.

  • Provide pediatric and adult primary medical care, dental care and behavioral health care to the medically underserved and uninsured families  of Louisiana.

  • Provide allied health care educational programs as well as preventive health care educational programs.

The Objectives:

  • Establish Community Health Centers.

  • Establish Federally Qualified Healthcare Center (FQHC).

  • Develop “New Access Points” for health care services in Louisiana.

  • Establish strong community collaboration and cooperation to address community identified needs.

  • Develop an identified network of resources.

  • Expand existing health care services to include medical, dental and mental health.

  • Develop prevention education programs within the healthcare centers. 

 

Meet Our Providers

Dr. Gary Chumley, D.D.S

Born in Greenwood, MS, graduating from Auburn with Bachelors in Science and proceeding his career in dentistry from LSU-SD in 1982.

Dr. Kenneth Bonnette, Jr., D.D.S

Ken is an undergraduate at LaTech University- 1974-78 and studied on a athletic scholarship for football. He graduated with a BS in Zoology and a minor in German language resulting to being put on the Deans List and Presidents List. LaTech is where I met my wife and having their 4 prodigies. 1978-1982 he attended Louisiana State University Medical Center- School of Dentistry. Then joined practice with my father-in-law, Charles H. Goben DDS 1982 thru 1995. After, worked at Willis Knighton Community Health & Education Center as the dental director, until  Nov 2010. Then worked with SMILES LA for about 6 months and along with Senior Dental Care ( nursing home dentistry- geriatric). Jan 2014 to present, he works part-time at Caddo Correctional Center and at CASSE dental since March 2018. He also recently signed up to complete a DENTAL EDUCATION CONTINUUM to further his knowledge of dental implants and plans to become a FELLOW OF I.C.O.I. ( International College of Oral Implantologists). His hobbies include: going to the beach with family, going to my in-laws property to deer hunt and manage the TREE FARM, going to small tract of land out in Waskom, Tx to ride 4 wheelers, and target practice with guns and bows. He also fishes and likes to travel and see historic sites in nearby states when I can with my wife or family.

Dr. Natalie McConnell, MD, FAAP

Dr. McConnell was born & raised in New Orleans, LA where she attended the Academy of the Holy Angels high school.  She went on to attend college at the University of Mississippi before moving to the Shreveport area for medical school at the Louisiana State University Health Sciences Center in Shreveport. She completed her Pediatric residency training here in 2002 & has been practicing pediatrics locally ever since. She is Board Certified in Pediatrics and has found it rewarding to educate new parents & watch her patients grow over the years. When not caring for her patients she enjoys spending quality time with her family & friends, as well as practicing yoga.

Patti Webb, FNP-BC

Patricia (Patti) Webb is a Board Certified Family Nurse Practitioner. She has over 30 years of experience in the medical field, with 17 years as a RN, and 13 years as a FNP.  Patti started her education in Nursing in 1988 at Louisiana State University in Baton Rouge, LA. She later transferred to Northwestern State University in Shreveport, Louisiana, where she graduated in 1988 earning her Bachelor of Science in Nursing.  Upon obtaining her RN, Patti went to work for Willis Knighton North ER, where she was employed for many years, eventually becoming the Clinical Educator and earning her certifications as an Instructor for ACLS, BCLS, PALS and NALS. After 2 years of teaching, she was promoted to Head Nurse of the ER. In 1993, Patti became a Flight Nurse for Life Air Rescue, an emergency medical helicopter that provided critical care transport for patients with critical needs.  As a team member and flight nurse of Life Air Rescue, Patti responded to numerous 911 calls/emergencies providing advanced, lifesaving skills and rapid transport to nearest hospital or Trauma Center.  She received advanced training in airway management as well as certifications in Advanced Trauma Life Support (ATLS), Pediatric Trauma Life Support (PTLS) and Emergency Medical Technician (EMT-B). Patti furthered her education in 2005 obtaining a Master’s Degree in Nursing from NSU in Shreveport and becoming a Family Nurse Practitioner. She joined our practice in July 2018.  She is happily married to “Captain” Brad Webb, a paramedic/firefighter with FD# 3 in Caddo Parish and has 4 children. Her favorite hobbies are spending time with her 3 beautiful grandchildren, reading, collecting antiques and gardening.  Patti truly enjoys her profession as a Family Nurse Practitioner and loves interacting with her patients as well as educating them concerning their illnesses and providing them the best care possible!

Dr. Naquita Webb, MD, FP

Dr. NaQuita L. Webb was born and raised in Plain Dealing, LA. In the fall of 2000, she graduated from Grambling State University with a Bachelor of Science Degree in Biology. After graduation, she and her husband, DeMarkus, moved to Dallas, Texas and she became a stay-at-home mom. She and her family returned to Louisiana in 2002, and she entered into medical school in 2004. She earned her Doctorate of Medicine from Louisiana State University School of Medicine-Shreveport in 2008. Upon completion of medical school, she was accepted into the Internal Medicine-Pediatrics residency program at Louisiana State University Health Shreveport where she completed her internship and residency in 2012. For the last 6 years, Dr. Webb has served patients of all backgrounds and ages via her work and dedication at Federally Qualified Health Centers here in North Louisiana. She has a passion for taking care of patients in underserved/rural areas. She welcomes patients of all diversities.

Courtney Brooks, APRN, FNP-C

Courtney is a Board Certified Family Nurse Practitioner, providing health prevention and promotion, as well as diagnosis of acute and chronic illnesses to patients from birth through the aging process. Born and raised in Monroe, Louisiana, Courtney obtained her Associate and Bachelor of Science in Nursing Degree from Northwestern State University, as well as her Master of Science in Nursing. She is currently an active member of the American Association of Nurse Practitioners and the Louisiana Association of Nurse Practitioners.

Aside from being a primary care provider, she is a new mother to her 8 month old daughter and enjoys spending time with her husband and step-son. The thing she loves most about her job is that she is provided the opportunity on a daily basis to form lasting relationships with her patients, while promoting their overall well-being.
